# Phase 2 Informed Consent Form — DRAFT
**Document version:** v1 (draft for review)
**Date:** May 18, 2026
**Status:** Pre-attorney review — DO NOT PUBLISH AS-IS
---
## ⚠ Important Notes Before Reviewing
This is a substantive first draft of the Phase 2 Informed Consent Form. It is structured to parallel the Phase 1 consent form but addresses Phase 2's significantly different scope: lifetime stewardship, recurring subscription, formal trust-based fund management, and strict behavioral commitments.
**Items marked `[BRACKETS]` require attorney input or DPO confirmation before publishing:**
- `[TRUST NAME]` — formal name of the legal trust entity
- `[TRUSTEE]` — identity of the trustee
- `[TRUST STATE]` — governing state law (likely Louisiana, matching Terms §19)
- `[TRUST TYPE]` — revocable / irrevocable / spendthrift / hybrid (attorney call)
- `[CANCELLATION POLICY DETAILS]` — what happens to trust balance / leased assets on cancellation
**Strong recommendation:** Have an attorney experienced in trust law AND research ethics review this draft before publishing. The combination of (a) recurring billing, (b) trust-based fund management, (c) strict behavioral commitments, and (d) restricted-spend research framework is unusual and benefits from legal review.

## 1. Hero + Step Indicator
> **Step 1 · Phase 2 Informed Consent**
>
> # Continuing as a DPO research participant — Phase 2
>
> Phase 2 is your voluntary, opt-in continuation of DoublePenniesOnline LLC research. It is a separate consent moment from Phase 1, with its own commitments and its own protections. This page walks you through everything Phase 2 involves so you can opt in with full understanding, or decide it isn't for you.
**Three-step indicator (matching Phase 1 visual):**
```
1. Read & Understand 2. Sign the Consent Form 3. Activate Phase 2
What Phase 2 involves Confirm your informed, Subscribe at $20/month
voluntary consent
```
**Why this order matters callout (gold integrity callout):**
> Phase 2 is a meaningful, ongoing commitment — to a recurring subscription, to behavioral expectations, and to a trust-based spending program. Real research at this depth begins with informed consent. Reading this page is your first act as a Phase 2 participant.
---
## 2. Eligibility Confirmation
Phase 2 enrollment is open only to participants who have completed Phase 1.
You're eligible to opt into Phase 2 if you have:
- ✓ Previously completed the Phase 1 Informed Consent Form
- ✓ Completed at least one Phase 1 research order
- ✓ Are not currently in the refund process for your Phase 1 order(s)
If you have not yet completed Phase 1, please begin there first: [/consent-dpo](/consent-dpo)
---
## 3. What You're Being Asked to Consent To
In short: by opting into Phase 2, you are subscribing to DoublePenniesOnline LLC's lifetime stewardship research program, agreeing to a recurring $20/month subscription, committing to maintain the Four Major Factors of stewardship, and accepting that your Phase 2 spending account is funded and managed through a formal legal trust under the DPO Exponential Growth Spending Model™.
### What your Phase 2 consent covers
- Continuation as a research participant in DoublePenniesOnline LLC's long-term stewardship study
- A recurring **$20/month subscription** that maintains your active Phase 2 status
- Access to Phase 2's restricted-spend categories (consumer spending — see Section 6)
- Acknowledgment that asset-based programs (Home, Auto, RV, Boat, Motorcycle Leasing) **require separate consent** when you choose to use them
- Maintenance of the **Four Major Factors** (work/volunteer, education, family values, community)
- Acceptance of trust-based fund management through `[TRUST NAME — TO BE IDENTIFIED]`, a `[TRUST TYPE]` trust governed by `[TRUST STATE]` law
### Phase 2 startup funding from Phase 1
At Phase 2 activation, your spending account will be initially funded based on your completed Phase 1 research orders. **The startup transfer is calculated at approximately $4,500 per completed Phase 1 order** (this amount and calculation method are subject to DPO verification and confirmation prior to your Phase 2 activation).
For example:
- 1 completed Phase 1 order → approximately $4,500 startup trust balance
- 5 completed Phase 1 orders → approximately $22,500 startup trust balance
- 10 completed Phase 1 orders (maximum) → approximately $45,000 startup trust balance
**Important caveats:**
- The specific dollar amount is **subject to confirmation** by DPO before Phase 2 activation
- The trust entity itself has **not yet been formally identified** — final trust details (name, trustee, governing state, type) will be confirmed prior to Phase 2 activation
- Your startup balance is non-cash, non-transferable, and held within the trust for restricted-spend use only
---
## 4. The Research Itself
### Research purpose
DoublePenniesOnline LLC's Phase 2 extends Phase 1's research into the **long-term, lifetime behaviors of stewardship-aligned households**. Phase 2 studies how participants apply structured spending support across qualified categories over years — not weeks — and how trust-managed funding can support household stability, dignity, and continuity.
### Procedures — what your Phase 2 participation involves
- **Phase 2 opt-in.** You voluntarily complete and submit this Phase 2 Informed Consent Form.
- **Subscription activation.** You authorize a recurring $20/month subscription that maintains your Phase 2 status. Payment is processed by DPO's third-party payment processor.
- **Spending account access.** Your Phase 2 spending account — funded through your Phase 1 trust accumulation and ongoing DPO Exponential Growth Spending Model™ mechanics — becomes available for use across approved Phase 2 categories.
- **Stewardship participation.** You maintain the Four Major Factors (see Section 8) as a condition of continued Phase 2 access.
- **Optional asset programs.** When you choose to use an asset-based Earned-Credit™ program (Home, Auto, RV, Boat, or Motorcycle Leasing), you complete a **separate consent** specific to that program.
- **Data collection.** DPO continues to collect category-level transaction data only — never individual merchant or itemized purchases.
- **Lifetime duration.** Phase 2 is designed for lifetime participation, subject to your right to cancel and the Four Major Factors maintenance requirement.
### Duration
Phase 2 has no fixed end date. Participation continues until:
- You cancel your $20/month subscription, OR
- You no longer maintain the Four Major Factors (resulting in loss of Phase 2 access per Section 8), OR
- DoublePenniesOnline LLC discontinues the Phase 2 program (with notice).
---
## 5. Your Rights as a Phase 2 Participant
These rights apply throughout your Phase 2 participation and afterward. To exercise any of them, email support@doublepennies.online.
### Right 01 — Voluntary participation
Phase 2 is opt-in. You may choose not to enroll. There's no penalty for declining, and your Phase 1 participation is unaffected.
### Right 02 — Right to cancel your subscription at any time
You may cancel the $20/month subscription at any time. Cancellation ends your active Phase 2 status. `[CANCELLATION POLICY DETAILS — what happens to your trust balance, accrued Earned-Credit on asset programs, and access to category programs.]`
### Right 03 — Data access & control
You can request a copy of any personal information DPO holds about you, request corrections, or request deletion (subject to legal retention requirements and trust-recordkeeping obligations).
### Right 04 — No retaliation
Cancellation of Phase 2 does not affect your eligibility for future research opportunities, your Phase 1 history, or your standing with DoublePenniesOnline LLC.
### Right 05 — Right to ask questions
You can email DPO at any time for clarification on Phase 2 mechanics, your spending account, the Four Major Factors, asset programs, the trust structure, or any aspect of this consent.
### Right 06 — Right to receive updates
You will receive operational communications about your subscription, your spending account, and any material changes to Phase 2 terms.
---
## 6. Phase 2 Covered Programs (Consumer Spending)
Your Phase 2 subscription provides access to the following restricted-spend categories. All transactions are tracked at the category level only.
### Life's Basics
Groceries, household essentials, utilities (electric, water, internet), basic transportation, qualified housing costs.
### Quality of Life
Approved discretionary spending aligned with stewardship principles — qualifying entertainment, dining, household enhancements within approved category guidelines.
### Education
Tuition, books, qualified educational materials, professional development, skills training programs for the participant and qualified household members.
### Financial Challenges Support
Emergency support categories for participants experiencing temporary financial hardship — `[SPECIFICS PER PROGRAM DESIGN]`.
### Low-to-Moderate Income (LMI) Households
Additional support categories specifically designed for participants whose household income qualifies under DPO's LMI definitions — `[SPECIFICS PER PROGRAM DESIGN]`.
### Consumer Debt Pay-Off
Qualified consumer debt repayment through the DPO Exponential Growth Spending Model™ funding structure — `[SPECIFICS PER PROGRAM DESIGN]`.
**Important:** All spending occurs through DPO's restricted-spend mechanism. Funds are never disbursed as cash to the participant. Category restrictions are enforced automatically at the point of transaction.
---
## 7. Asset-Based Programs (Separate Consent Required)
Phase 2 also offers asset-based programs operating under DPO's Earned-Credit™ research model:
- Home Leasing Earned-Credit™ Program
- Auto Lease Earned-Credit™ Program
- RV Leasing Earned-Credit™ Program
- Boat Leasing Earned-Credit™ Program
- Motorcycle Leasing Earned-Credit™ Program
**These programs are NOT covered by this Phase 2 Informed Consent Form.**
Each asset program has its own dedicated consent process that you complete at the time you choose to enroll in that specific program. Those consents address:
- The Earned-Credit™ mechanism specific to that asset class
- The lease structure and your participant rights
- The asset acquisition and ownership/usage rights
- Cancellation, asset disposition, and accrued Earned-Credit treatment
- Risks specific to that asset class
Opting into Phase 2 does **not** automatically enroll you in any asset program. You can use Phase 2's consumer spending categories without ever using an asset program.
---
## 8. Behavioral Commitments — The Four Major Factors
Phase 2 participation is conditioned on maintaining stewardship through four expected practices. **These are strict requirements, not aspirational guidelines.**
### Factor 1: Work or Volunteer Service
You commit to at least **40 hours per week** of paid work or qualified volunteer service. Documented exceptions apply for:
- Medical/disability limitations (with appropriate documentation)
- Caregiving for infants (parents of children under daycare-eligible age)
- Other circumstances qualified per `[DOCUMENTATION POLICY]`
### Factor 2: Education or Skills Development
You commit to **ongoing education or skills development** — formal coursework, professional development, trade learning, lifelong skills, or qualifying self-directed learning. Participants are expected to be growing in capability over time.
### Factor 3: Family Values
You commit to **stewardship-aligned family values** in your household: supporting family members, contributing to household stability, and conducting yourself in alignment with DPO's stated principles of reliability, empathy, and shared responsibility.
### Factor 4: Community Participation
You commit to **active community participation** — respecting the broader Phase 2 participant community, contributing positively to local community life, and not engaging in conduct that undermines the integrity of the program for others.
### Consequence of non-maintenance
**You understand that failure to maintain the Four Major Factors may result in loss of Phase 2 access.** DPO will provide reasonable notice and an opportunity to address gaps before terminating Phase 2 access for failure to maintain these factors. Documented exceptions and good-faith effort will be considered.
---
## 9. Subscription Terms
### The $20/month Phase 2 subscription
- **Amount:** $20 USD per month
- **Billing:** Recurring monthly via DPO's third-party payment processor
- **Authorization:** By completing this consent, you authorize DPO to charge $20/month to your provided payment method until you cancel
- **Cancellation:** You may cancel at any time via your account or by emailing support@doublepennies.online
- **Cancellation effect:** Cancellation ends active Phase 2 status. `[POLICY DETAILS — what continues vs. what ends, any prorated treatment, what happens to spending account balance, what happens to ongoing asset program participation]`
- **Failed payments:** If your monthly payment fails, DPO will attempt to contact you and may suspend Phase 2 access until payment is restored
- **Subscription changes:** Material changes to the $20/month subscription terms require advance notice and an opportunity to cancel before changes take effect
---
## 10. Trust Mechanism
Your Phase 2 spending account is funded and managed through a formal legal trust entity. **The specific trust has not yet been formally identified.** Final trust details — including the trust's name, trustee identity, type, governing state law, and your relationship to the trust — will be confirmed prior to your Phase 2 activation.
### Trust details (to be finalized prior to activation)
- **Trust entity name:** `[TRUST NAME — TO BE IDENTIFIED]`
- **Trustee:** `[TRUSTEE NAME — TO BE IDENTIFIED]`
- **Trust type:** `[TYPE — revocable / irrevocable / spendthrift — TO BE DETERMINED]`
- **Governing law:** `[STATE — likely Louisiana matching DPO's Terms §19, TO BE CONFIRMED]`
- **Your relationship to the trust:** `[BENEFICIARY / OTHER — TO BE DETERMINED]`
DPO will provide written notification of finalized trust details to all Phase 2 participants before activation. Participants will have the opportunity to review the finalized trust details and decline Phase 2 activation if the finalized terms differ materially from what is described here.
### Phase 2 startup funding from Phase 1
At your Phase 2 activation, your spending account will be initially funded by a transfer based on your completed Phase 1 research orders:
**Approximately $4,500 per completed Phase 1 order** (subject to DPO verification and confirmation prior to activation).
This startup amount represents the trust-accumulated value generated during your Phase 1 participation through the DPO Exponential Growth Spending Model™. It is the foundation of your Phase 2 spending capacity.
| Phase 1 orders completed | Approximate startup trust balance |
|---|---|
| 1 order | ~$4,500 |
| 3 orders | ~$13,500 |
| 5 orders | ~$22,500 |
| 10 orders (Phase 1 maximum) | ~$45,000 |
The exact amount transferred to your trust account will be confirmed by DPO before Phase 2 activation and will be reflected in your Phase 2 onboarding documentation.
### Ongoing trust funding
Beyond your startup balance, your Phase 2 spending account is also supported by:
- **Ongoing DPO Exponential Growth Spending Model™ mechanics** that maintain the trust's overall funding capability
- Your continued $20/month subscription, which supports the trust-managed research environment
### How the trust works
The trust:
- Holds the funds in a fiduciary capacity
- Disburses funds only in accordance with Phase 2's restricted-spend rules
- Is bound by trust law to administer the funds for the stated purpose
- Is subject to applicable trust accounting and reporting requirements
You understand that:
- Your Phase 2 spending account is **not your personal cash account**
- Funds are not transferable, not withdrawable as cash, and not available outside the Phase 2 restricted-spend mechanism
- The trust structure is what enables Phase 2's research-protected, restricted-research-value framing
- `[CANCELLATION DETAIL — what happens to your trust balance, including your startup balance, if you cancel Phase 2]`
---
## 11. Risks, Benefits, Data, Tax, and What Phase 2 Is NOT
### Risks
- **Financial:** Your $20/month subscription is at risk if you cancel before deriving full value from Phase 2. Subscription fees paid for past months are not refundable.
- **Behavioral:** Loss of Phase 2 access is a real possibility if you do not maintain the Four Major Factors. You may lose access to your spending account and any associated benefits.
- **Trust risk:** Your trust balance and spending account access depend on DPO's continued operation, the trust's continued solvency, and the DPO Exponential Growth Spending Model™ continuing to function as designed. DPO makes no guarantee of perpetual operation.
- **Asset program risk:** If you opt into asset-based programs separately, those programs carry their own substantial risks disclosed in their respective consents.
- **Privacy:** Category-level transaction data continues to be collected. Individual purchases remain unseen by DPO.
### Benefits
- **Long-term household stability:** Access to restricted-spend categories that support steady household provisioning across years.
- **Trust-protected value:** Funds are sheltered within a formal trust structure designed to protect their intended use.
- **Stewardship community:** Membership in a defined community of participants committed to the same principles.
- **Research contribution:** Your continued participation generates research outcomes that may benefit other participants and the broader stewardship-aligned community.
- **Optional asset access:** Eligibility to enroll separately in Earned-Credit™ asset programs.
### Data and confidentiality
Same standards as Phase 1, extended:
- **What's collected:** Account information, recurring subscription billing reference, category-level transaction data
- **What's NOT collected:** Individual merchant data, itemized purchase data, personal financial information beyond what's required for trust administration
- **Protection:** Encryption in transit, limited internal access, third-party processors bound by contract, no sale of personal information
- **Trust recordkeeping:** The trust maintains records of disbursements to satisfy fiduciary and tax obligations
### Tax clarity
Because your Phase 2 spending is:
- Restricted, non-cash, trust-protected
- Usable only for approved Phase 2 categories
- Part of a documented long-term research experiment
- Disbursed through a formal trust structure
…the value you use in Phase 2 is designed not to function as personal income or compensation.
**DPO does not provide tax, legal, or financial advice.** Participants are responsible for understanding their own personal tax situations and should consult a qualified professional with questions specific to their circumstances. The trust's tax treatment is also subject to applicable trust tax law.
### What Phase 2 is NOT
- Not income, compensation, or wages
- Not a payout, loan, or investment
- Not a securities offering or financial product
- Not a multi-level marketing or recruitment program
- Not a guarantee of perpetual benefits
- Not a substitute for personal financial planning
- Not financial, legal, or tax advice
- Not an entitlement program
Phase 2 is a **trust-funded, restricted-spend, behavior-conditioned research program operating under NAICS 541720** — Research and Development in the Social Sciences and Humanities.
